The typical scenario
Employee reports an injury, sees their GP, is signed off work, or stays at work and is less productive (presenteeism). In the UK, the NHS waiting list for physio is 10-14 weeks, hence conditions become chronic, and employees often develop mental health complications (anxiety, stress, depression).
If absent, the employee takes sick leave for 14 days (average), increasing workload for their colleagues, line manager, HR and (possibly) occupational health.
The indirect costs most organisations don't calculate: Management time spent on absence conversations, return-to-work interviews, and phased return coordination, conservatively 3–5 hours per episode. Lost productivity during the weeks before absence, when the employee was present but performing at a fraction of their capacity. Team disruption. The colleagues covering, adjusting, and absorbing the gap. Recruitment and retraining if the absence tips into capability, with an average cost of replacing an employee sitting between 50% and 200% of their annual salary. Repeat episodes of MSK conditions that aren't properly resolved have a high recurrence rate, meaning the cost isn't a one-off.

Employee reports a back injury, is immediately triaged and directed to self-help advice, support, and treatment or rehab if required.
Remaining at work is encouraged, and automated follow up such as nudges, outcome measurements and exercise upgrades keep the patient engaged with self-management without any effort from clinicians.
